SDOH stands for Social Determinants of Health. What does that mean exactly? You can find various definitions as you research this topic. I like the World Health Organization (WHO) definition: “the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life.” The individual’s personal circumstances can be impacting their health and well-being. SDOH can be found within five groups:
a. Healthcare
b. Education
c. Economic Stability
d. Social and Community
e. Neighborhoods
Each of these groups will be explored in detail. Michigan’s Roadmap to Healthy Communities will be introduced and reviewed. Example of how SDOH is addressed within our FPC on a regular basis.
